Fee Schedule

We accept BLUECROSS BLUESHIELDAETNAHUMANA,  and CIGNA insurances. It is your option whether to seek reimbursement from your insurance company for our appointments. We provide “superbills”, when requested, which you can then submit to your insurance company for out-of-network reimbursement as their policy allows.

If you are eligible for Medicare or are presently receiving Medicare medical benefits, please be aware that we have opted out of billing Medicare. In other words, you will not be able to send statements for Care Bridge Psychiatry to Medicare for reimbursement. Payments are due at time of service. For your convenience, our office accepts most major credit cards.

  • Initial Evaluation (75 minutes)
    -Consists of comprehensive mental health, medical, social, developmental and family histories.  You will be requested to review and complete the patient registration and consent forms in the Forms link.  Either fax or bring the completed forms with you, or plan to arrive 20 minutes prior to your scheduled appointment.
  • Psychotherapy +/- medication management (60 minutes)
    – Therapy consists of an initial period of gaining understanding and a mutual trust and focus.  We then work to increase insight and challenge those patterns that continue to cause dysfunction.  Dr. Umoren will work with you to determine the optimal frequency of these appointments, although they commonly occur on a weekly or twice-weekly basis.  Therapy may be short-term or long-term in nature.
  • Psychotherapy +/- medication management (45 minutes)
    – Intermittent appointments to monitor the effect of psychiatric medications will continue for as long as medications are prescribed.
  • Medication Management (20-30 minutes) 
  • Telephone consultations -5 mins
  • Refill of controlled substance without session 
  • Prescription request on weekend 
  • Letters/Forms for insurance reimbursement 
  • Copies of medical records 
  • Buprenorphine induction 
  • Buprenorphine follow up visits
